ADDITIONAL TEST RESULT REQUEST FORM

Requests for additional test results must be made via this form only. Requests by telephone, email, or fax will not be accepted. Additional Official Test Results are sent electronically to the requested recipient. Before completing this form, please review The CELBAN Centre’s Policies & Procedures.

To request an additional copy of an Official Test Result, please complete and submit the form below. The cost of each additional test result is CAN$ 35.
The fee will be collected using PayPal. You will not need to sign up for a PayPal account to do this.

Upon completion of payment, you will receive an email confirming the request.
Please note that The CELBAN Centre does not send Official Score Reports to examinees. Official Score Reports are sent only to institutions or licensing bodies directly by the CELBAN Centre.
Please allow up to 5 business days for the additional Official Test Result to be sent to the recipient requested. To avoid delays, please submit the form only once and ensure all information is correct.
